What you need to do to order by mail is to follow the link below which says mail order form. Please read the statement completely.
You will need to send me your full name printed out plus your full signature & birthdate.
I need a short statement that you agree to the statement that you read on the mail order page.
The current date as well as the DVD selections you wish to have sent for your order. Don't forget your mailing address for shipment.
